Machines may differ, but it seems that if you start sewing without hooking the upper & lower threads without fabric there, the upper may be OK or bad but the lower a knotted mess. The ka-chunk sound gives it away. Just did the pull test and all good, but started sewing without that 1st hook. Then cut the threads, and ran again on different fabric section and was OK. Maybe auto-threading machines handle this, but this is a Bernina 801 scored resale in 2020. I think this is a different problem than the tension one, looks different than the above pic.

This is the first time I’ve ever actually grasped how a stitch is formed by the machine- it’s only taken 50 years to work it out! Brilliant! Thanks

And I thought I invented the pull test. Lol. I trouble shoot “machine” issues where I work. This is the most common problem and solution. Well done!
